The REACH Transplant (Renal Education and Choices @ Home) programme is a Scottish Government initiative to improve equity of access to living donor kidney transplantation (LDKT) in Scotland.
This initiative uses an evidence-based approach of individualised, home education sessions to provide tailored information about living donor kidney transplantation (LDKT) to suitable chronic kidney disease (CKD) patients and their support networks.
This personalised approach is intended to overcome known barriers to accessing living donor kidney transplantation as a first treatment option for those who would benefit from transplantation. The postholder does not recruit potential living kidney donors but enables informed decision-making by patients and members of their support network.
The postholder will be employed in their local renal unit with professional links to the the REACH Transplant Programme Lead who acts as the professional support to the REACH Transplant Nurse Specialists in renal units in Boards throughout Scotland.
The postholder will be required to provide regular audit outcomes and reports to the Programme Lead for the Scottish Government.
Knowledge, training and/or experience to do the job
Registered Nurse with valid NMC Registration
Educated to SCQF Level 10 post graduate qualification for example clinical decision making or specialist renal/transplant module.
Experience of working in renal/transplantation
Knowledge of principles of patient education.
1x 15 hour / permanent post
