This is a fixed term post of 2 years.

The shift pattern for this post is five days out of seven.

Opportunity to join a great pharmacy team in primary care in the North West Locality Glasgow City HSCP.

This is an exciting opportunity for an enthusiastic and motivated pharmacist to lead on the delivery of medicines for the elderly, this includes patients receiving home care and those in the care home setting in North West Glasgow. This role will be patient facing and will involve taking on a prescribing role and developing your own clinical caseload in line with the Achieving Excellence in Pharmaceutical Care Strategy.

There will a focus on working with the MDT to deliver pharmaceutical care and application of Realistic Medicine to ensure patients get the best possible outcomes from their medicines, avoid harm & waste and prevent unplanned admissions to hospital. This will require close working with the wider HSCP primary care team as well as linking with services such as the Care Home Collaborative and care home nursing teams.

As a member of the North West pharmacy team this role will involve leading on aspects of service development and quality improvement, working with complimentary post holders in North East and South Glasgow Localities to shape the strategic direction of medicines management delivery in the frail elderly across Glasgow City HSCP. In addition you will be required to work with the HSCP care home commissioning team, social work colleagues and care providers to implement service change.

You will require to be good team worker, good clinical skills and have experience of leading on service developments and change management and possess excellent communication and organisational skills.

Minimum requirements Masters Degree in Pharmacy or equivalent, Registrant with the General Pharmaceutical Council,​​​​​​​ Qualified as an Independent prescriber (? Post registration qualification or equivalent experience)
