Band 7 (£40,872 - £47,846) - 37.5 hours per week 

The aim of the In Reach Heart Failure Nurse is to improve patient’s quality of life, reducing unnecessary hospital admissions and readmissions by providing high quality holistic, evidence based care. The post holder will provide an effective link between primary and secondary care, provide education and support to patients and carers.

Based within Aberdeen Royal Infirmary, the post holder will have a clinical responsibility for the assessment, planning and co-ordination of care to people with diagnosed heart failure. They will support patients with a diagnosis of heart failure in hospital and facilitate and effective discharge planning to provide continuity of care into the community. By managing their own caseload, providing personalised care plans, reviewing medication and lifestyle changes through education and support to patients, the aim will be to improve patient’s quality of life, reducing unnecessary hospital admissions and re-admissions.

Heart failure nurses work to an advanced competency level (based on nationally recognised educational pathways developed by the British Society for Heart Failure). This post holder will link with Medical and Cardiology consultants within the adult inpatient areas, multi-disciplinary colleagues in primary care, General Practitioners and palliative care teams. The successful candidate will work as part of the wider Cardiology Nurse Practitioners team and will deliver and develop this service over a 2 year period.

Cardiology is a continually evolving service and there will be ongoing developmental opportunities across the service. The In Reach Heart Failure Nurse will work alongside a large team of Nursing, Medical, Allied Health professionals and other specialist services to provide support to patients across NHS Grampian. Further training and development is available to enable delivery of patient care in a dynamic and supportive environment.
