7.5 to 11.5 hour shifts (variable) Monday - Sunday inc. public holidays
As part of a multidisciplinary team the post holder will have responsibility for the assessment of baseline care needs for complex vulnerable
frail elderly patients. This will include the development and implementation of high quality programmes of care in conjunction with the
multidisciplinary team, regarding the best management of these patients.
The post will include assessment of presenting conditions, initiation of investigations, determination of differential diagnosis, checking
results and initiation of treatment. Where appropriate, this may involve facilitating discharge of patients, initiating onward referral and
liaison with the primary care team The post holder will play a significant role in the education of ward staff regarding the management of
complex frail elderly patients, while keeping up to date with evidence based practice. This will include monitoring and evaluating standards
of care with the specialty area, as well as championing on audit and change processes Minimum requirements : Registered Nurse with 5 years’
experience, 2 years of which were within relevant specialty. Educated to or working towards degree level. Evidence of further education
including post graduate certification / diploma/ continuous professional development in relevant area of practice.
The post holder will be required to demonstrate excellent team working skills with ability to work using own initiative. Effective listening,
communication and interpersonal skills are essential. Ability to work with people and as part of a multidisciplinary team.
Effective time management skills.
