NHS Tayside: Band 6, Permanent   £33,072 – £40,736 per annum

The Sources of Support social prescribing service is a collaboration between Dundee Health and Social care Partnership, Primary Care and Scottish Government. It sits within the Memorandum of Understanding as one of the national priorities for development through the Primary Care Improvement Plan and Action 15 developments in mental health. 

 

An exciting opportunity has arisen for an experienced candidate to take on the role of link worker in identified GP Practices in Dundee. Link workers provide person-centred support to referred patients to help them identify priorities and outcomes and access a wide range of non-clinical services and activities to improve health, wellbeing and life circumstances. 


Candidates must be autonomous practitioners with previous post qualifying work experience of 1 to 1 case management with people who have complex social and emotional circumstances; a strong understanding of the challenges faced by people living in deprivation; community based, person-centred, non-clinical approaches; and collaborative, multi-disciplinary working. Applicants must be educated to degree level in a relevant area, preferably with evidence of post-graduate training or qualifications. It is desirable that applicants have previous experience of working within social prescribing services.


You will be required to achieve the mandatory Induction standards for Healthcare Support Workers in Scotland and to comply with the Code of Conduct throughout your employment.
