​​​​​​​​​​​​​​A new, exciting opportunity has become available within Dumfries and Galloway Health and Social Care Partnership for a Care Home AHP Lead.
The post holder will act as the facilitator to co-ordinate and support the implementation of universal, targeted and specialist interventions for care home residents within Dumfries and Galloway. There will be a focus on early intervention and maximising independence.  As Care Home AHP Lead you will provide highly specialist clinical input and professional leadership to care homes across the region. 
Working closely with the Community Lead AHP, Care Home Tactical Team, Community Beds and Supported Living Division and Home Teams the post holder will ensure that our care home residents receive effective safe high quality care and our care homes and their staff are supported to deliver this. 
We are looking for a new member of the Senior AHP Team to bring their enthusiasm, energy, strong leadership skills, expert knowledge and skills.
