Are you an experienced registered Physiotherapist seeking a new and exciting challenge in an integrated team? There is an exciting opportunity within the WL Hospital at Home Team for an APP to join the team. This is a new post and a key role in developing respiratory services to support hospital discharge and develop community services.
To undertake the role of an Advanced Physiotherapy Practitioner (APP), utilising high levels of decision making and clinical judgment, ensuring that patient health needs are met through the diagnosis and the management of complex patient conditions including assessment, treatment and review planning processes in partnership with patients and families.
Provide specialist advice to healthcare professionals, other agencies, carers, clients and relatives. The APP role encompasses that of expert clinician, educator and auditor.
Take a key role in leading and developing the service to improve practice and work collaboratively, implementing and evaluating evidence based standards, guidelines and policies. To identify and be actively involved in audit and research activity which will contribute to the development of the service.
The successful applicants will be joining a dynamic multi-disciplinary team with a passion for integrated working and putting patient outcomes and experience and the heart of their practice.
