An exciting opportunity has arisen for a band 6 specialist Physiotherapist to the Rhins Home Team. This post would offer an opportunity to work as part of the newly developed home team to support patients at home and aid admission prevention and discharge without delay.

Can you inspire, enable and empower individuals, Carers and colleagues through compassionate conversations to work collaboratively to achieve what matters within improved health and wellbeing outcomes?  Can you demonstrate your skills through re-treatment programs, supporting self-management, support mental well-being and maximising independence?

Can you confidently self-coordinate a complex caseload whilst proactively seeking shared best decisions with individuals and multi-disciplinary multi-agency partners?   Can you demonstrate effective working relationships across Locality Teams inclusive of GPs, Nursing, Social Work, Social Care, AHPs, Third and Independent sector organisations?   Are you aware of Home Team early adopters and how you will be contributing towards working differently in the future to achieve transformational improvements for the health and well-being outcomes for the people of Dumfries and Galloway?  It is a very exciting time with transformational change as we move forward with the development of the pathways within rehabilitation and reablement.
