The shift pattern for this post is 9am till 8pm in a shift pattern, 5 over 7 days.
This post will be part of the Rehabilitation and Enablement service providing an enhanced response to support for patients being discharged from the Home First Response Service as part of an MDT approach. The successful candidate will have experience within a care environment, have or working towards SVQ level 3 and be able to work within a MDT team to provide hands on care and support within a rehabilitation ethos. This post offers the opportunity to develop competencies across all health fields to be able to support nursing and AHP care plans, treatments and therapies as well as the opportunity to work closely with patients to enable them to live safely at home.
Hours of work will be flexible to cover between 8am and 8pm, 7 days per week. Referrals to the team will be from acute sites to aid a speedy and safe discharge, from GP and primary care with the aim of keeping people at home.
