 Please note this is a two year fixed term post Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have written agreement from their line manager to be released on a secondment before applying for the post.  

An exciting opportunity to contribute to the Discharge without Delay agenda across NHS Lanarkshire, this post will support the safe transition of patients with complex moving and handling requirements from the acute to community setting. Working alongside the discharge facilitators, nursing and AHP teams, the post holder will act as an expert moving and handling discharge advisor, supporting early decision making, equipment and care provision and environmental considerations to align with patients’ planned date of discharge.

The post holders will carry a clinical caseload in addition to educating and supporting the multidisciplinary teams across the acute hospitals, offsite beds and community settings in South Lanarkshire.

KNOWLEDGE, TRAINING AND/OR EXPERIENCE REQUIRED TO DO THE JOB

Registered Nurse / AHP / Pharmacist with valid NMC/HCPC / GPHC Registration.

Educated to working towards/operating at Masters Level.

Post Graduate Diploma in Advanced clinical practice and expertise within speciality 

area of practice that demonstrates the required breadth of knowledge required to lead safety, effectively and efficiently. 

Comprehensive knowledge of clinical guidelines and standards within the speciality.

Advanced Specialist knowledge and experience of relevant conditions, pathology, policies and procedures associated with the speciality/area of practice ensuring that the level of expertise can be utilised to deliver leadership within speciality.

Evidence of application of advanced practice skills in speciality and applying clinical judgement. ​​​​​​​
