We have an exciting opportunity for a Registered Nurse to join our Frailty Assessment Unit, based at University Hospital Monklands . 

The successful candidate will be responsible ​​​​​​​for the assessment of care needs and the development of programmes of care, and/or the implementation and evaluation of these programmes, ensuring the delivery of high quality care to patients. The post holder is expected to carry out all relevant forms of care without direct supervision and may be required to demonstrate procedures to and supervise registered and/or unregistered staff, as well as student nurses.

The Frailty Assessment Unit is a 24 bedded ward which is used to take patients directly from Accident and Emergency. The successful candidate will carry out comprehensive geriatric assessment of patients identifying Medical, Psychosocial and Functional Limitations of an elderly person. The post holder will develop a coordinated plan for the patients return home or make the decision for the patient to remain in hospital, in one of the cote wards.​​​​​​​​​​​​​​

Please note the successful candidate must:

  • Be a Registered Nurse with valid NMC registration 

Knowledge, training and / or / experience required to do the job:​​​​​​​

  • Excellent Team Working skills with the ability to work using own initiative​​​​​​​​​​​​
  • Effective listening, communication and interpersonal skills
  • Ability to work with people and as part of a multidisciplinary team
  • Excellent time management skills
  • Working knowledge of basic information technology​​​​​​​
