​​​​​​​​​​​​​​As part of the Scottish Government Discharge Without Delay agenda within South Lanarkshire work has commenced on developing multi-professional, multi-agency Home First Teams. Phase 1 of this process has produced very exciting outcomes in respect of delivering on this agenda through maximizing function, reducing deconditioning, preventing hospital admission where appropriate, supporting people to return home as quickly as possible where hospital admission has been required, and delivering on the issues that people tell us really matter to them to enable them to live their lives in the way they want to. Interventions focus on input within the community setting as well as actively promoting discharge from the in-patient sites.


Following the very positive first phase of these developments, we are now moving to phase 2 of the process to further expand the initiative across the area.


We have exciting opportunities for enthusiastic, focused, dedicated, flexible people to join our developing Home First Teams.
As part of a multi-disciplinary team, the post holder will manage their own specialist caseload, using evidence-based practice and client-centred principles to assess, plan, implement and evaluate interventions in both hospital and community settings. Provide Physiotherapy advice to staff and other disciplines/agencies. The post holder is expected to develop specialist clinical skills and may lead clinical audits and undertake research.
Duties may vary depending on the care group/department / assigned area of work or level of experience.
A full valid UK driving licence holder would be desirable for this position to meet the needs of the service,


The following KNOWLEDGE, TRAINING, AND/OR EXPERIENCE  will be required to do the job:


Registered Physiotherapist with valid HCPC Registration
Educated to/working towards/operating at Degree Level
Comprehensive knowledge of clinical guidelines and standards within the specialty/area of practice.
The post holder will possess excellent team-working/leadership skills and have the ability to motivate others and work using their own initiative
Excellent listening, communication, and interpersonal skills.
Effective time management skills
Working knowledge of basic information technology
