Band 6 Community Learning Disability Nurse – Assertive Outreach Model
A full time, permanent Band 6 Community Learning Disability Nurse is required for a new Assertive Outreach model within the Renfrewshire Learning Disability Service (RLDS). This exciting new post is an addition to the existing nursing team and will work collaboratively with the other nursing colleagues and the wider multi-disciplinary team under the same Operational Management structure.
This is an exciting progressive outreach model currently being developed within the team which will focus on crisis management and intensive interventions to complex individuals who access our service. These individuals may be at risk of emerging crisis within their own home environment or community placement’s, and in alternatives to hospital admission. Part of the role would also ensure those who have recently been discharged from hospital can successfully transition back into the community with intensive support aimed at all stakeholders, families and wider services. A strong focus on person centred behavioural support methods is key to manging safe and positive outcomes in the community.
The Post holder will maintain close working relationships within the MDT, Specialist Learning Disability Inpatient Services and all stakeholders with the ability to work in collaboration and/or signpost to other services as required. Modelling appropriate behavioural management strategies and implementing risk management plans will be a critical part of this role.
Prior experience in Learning Disability and Mental Health assessment and intervention is desirable. It would be advantageous that the post holders has previous knowledge or sought professional development in complex trauma and individuals who exhibit behaviour perceived as challenging.
The Renfrewshire Learning Disability Service operates between the hours of 9am to 5 pm Monday to Friday. This exciting new model of assertive outreach could by exception require the post holder to work beyond those hours.
Applicants should have professional registration, 1st degree Level Qualification in Learning Disability Nursing.
