An exciting opportunity has arisen for a band 6 Community Mental Health Nurse (CMHN) to join an established and welcoming team. We are looking for an experienced nurse who has a working knowledge of community services. The post holder will work within a generic community mental health team (CMHT).

The post holder will work with colleagues in the CMHT and the wider mental health network to ensure that service users are offered the most appropriate service available to meet their needs. The post holder will demonstrate skills in risk assessment and risk management. The post holder will deliver a range of mental health assessments, treatments, education and support. The post will predominantly entail working with older adults who require memory assessment and appropriate follow up care within the dementia pathway. The post will also require working with adults age 18-65 with mental health problems. 

Team priorities include physical health parity of esteem, collaborative working, triangle of care and patient self management. The successful candidates will be based within Mountainhall treatment centre and will be expected to travel within a specified area whilst working in partnership with service users, carers, families and other agencies across the locality. The post will provide opportunity for personal development and growth in a friendly, experienced and supportive team. Full supervision and mentoring will be provided.
