The shift pattern for this position is 0800-1600.

An exciting opportunity has arisen for a Surgical Elderly Care Assessment Nurse to assist in assessing and streamlining the safe discharge of our Elderly Care patient group.

You will be responsible for the comprehensive assessment of our Surgical patients in the over 65 group, to facilitate the safe and timely discharge post operatively. You will be able to work autonomously and liaise with our AHP Rehab teams and prepare the relevant discharge packages where applicable.

You will have a defined caseload of patients across our General Surgery specialty for whom you provide holistic person centred care to throughout their journey. You will be directly responsible to the Band 7 Senior Nurse Practitioner/Educator and the Lead Nurse for General Surgery, Vascular and Breast.

You will be fundamental to service development and collaborative working with the whole Surgical team, ensuring that this patient group receives the right care and interventions at the right time every time.
Ideally you will be a registered nurse with degree or equivalent and evidence of significant experience within this area. Evidence of involvement with Quality Improvement initiatives is desirable but not essential as we look to drive forward improvements in this area.

Sound clinical knowledge and significant post registration experience is also essential.​​​​​​​
