West Dunbartonshire has a diverse population of approximately 90,000 people and covers two distinct localities. West Dunbartonshire Health & Social Care Partnership [HSCP] employs approximately 2083 staff and manages the services of various client groups. The HSCP has established links with the Acute Sector, the Third Sector and a number of independent contractors. The Health & Community Care Services within West Dunbartonshire HSCP offers a service to individuals and their carers through a coordinated approach by all Health & Social staff.
We currently have a permanent full time vacancy for a Community Occupational Therapist within the Adult Care Team. This post offers the opportunity to work as a specialist Occupational Therapist for adults aged 16-65 years of age. The post holder will be part of a multi-professional team contributing to patient/client care. The post holder will deliver a high quality specialist service, provide expert clinical intervention, education and support to patients/clients and their carers.
The post holder will provide specialist assessment, treatment and case management for adults with complex rehabilitation needs, they will also support any equipment or adaptation that has been assessed for as a need.
The post holder will participate with Anticipatory Care Planning as required with the aim of minimising avoidable hospital admission and maximise an individual's ability to remain safely at home or in a homely setting.
Council Salary: £39,125.86 - £42,355.93 (Grade 8) - 35 hours per week
