NHS Borders are delighted to invite applicants to join our Mental Health Older Adults Service - Post Diagnostic Support team (MHOAS- PDS) as a Band 3 HCSW.

MHOAS- PDS aim to deliver person centred, goal orientated and strength based care for individuals following being given a diagnosis of Dementia. Your role would include supporting the team with delivering 5/ 8 pillar model of Post diagnostic support to individuals within the Scottish Borders.

Contributing in MDT discussion, case load discussion and management, theraputic interventions both for individuals and group work, Collaborating with other services to raise awareness/ joint working and information sharing to benefit individuals

We are seeking a highly motivated HCSW  preferably with experience of working with older people living with Dementia, who can demonstrate confidence, knowledge, participate in ongoing learning and development, team working and good communication skills.

We are a close, supportive and inclusive team with high standards for delivery of clinical services within the Scottish Borders
