An exciting opportunity for an experienced nurse or who is either an advanced nurse practitioner or who is willing to work towards and advanced nurse practitioner to join the North Intermediate Care & Rehabilitation Services.  You will be expected/ required to provide expert clinical care and management of both the stable and acutely unwell older person, making complex, autonomous decisions using advanced knowledge and skills, having overall responsibility for the assessment of care needs and the delivery of advanced nursing care to personal caseload.  

You will work collaboratively with the GP with a speciality interest and the multidisciplinary team within an Intermediate Care and Rehabilitation hub on the Ayrshire Central site, as part of the North Ayrshire Health and Social Care partnership, to promote early discharge to a homely setting, to respond to existing or potential ill health issues in the person’s home/homely setting, proactively identified by local GP’s, and to contribute to a reduction in unplanned acute hospital admissions.

You will be expected to perform clinical assessment, initiate investigations, determine differential diagnosis and undertake appropriate first line interventions for the unwell adult (supported by a structured competency-based framework and intermediate care GP).

Hours of duty : 37.5 hour per week
Permanent position 

You will be an expert within the field of older people/adults with complex needs with highly developed specialist knowledge.  You will be required to have a Registered Nurse qualification with current NMC registration. You will also require:-

•    Specialist post-graduate/post-registration qualification (or equivalent experience) in an older adult/adult with complex needs related discipline
•    Demonstrable competencies in older adult assessment and interventions
•    Evidence of continued professional development, and other relevant short courses
•    Non-medical prescribing qualification
•    Masters level short courses related to advanced nursing practice or a willingness to work towards.
•    A commitment to lifelong learning and demonstrates evidence of continuing professional development
•    Evidence of management education and training
•    Ability to keep relevant skills / knowledge updated and documented
•    The post-holder will be required to demonstrate excellent communication / team working skills with ability to work using own initiative
