Band 6 Community Dietitian- Frailty – Aberdeen H&SCP
Base: Links Resource Centre, City Hospital, Aberdeen AB24 5AU (opportunity for remote working)
Do you have excellent communication skills, an ability to work in partnership with others, a patient focused approach and a desire to improve services to meet patient needs? Then come and join our community dietetic team in Aberdeen City.
This is a relatively new post and offers an exciting opportunity to work with other members of the frailty team to lead, develop and deliver a new service to patients who are considered part of the frailty pathway, offering services to patients in clinics, care homes and their own home.
This post will be supported by a HCSW and a dietitian already working in frailty.
We are actively engaged in the use of technology to deliver care wherever possible and there will be opportunities to develop this further. There is an opportunity for the post holder to work almost entirely remotely and to deliver clinical services to other patient groups such as diabetes, adult weight management, GI, children.
We have well established links with dietitians across all sectors in Grampian and work together to provide a range of CPD and development opportunities, particularly around clinical effectiveness, patient safety and patient centeredness.
The postholder will participate in undergraduate student training and training to a range of key stakeholders across the partnership.
You must be a car driver, hold a full driving licence.
For AHP posts within Grampian, there is a requirement to provide services flexibly to meet the needs of service users. This will include providing services over weekends and extended hours when necessary.
