An exciting opportunity for a highly motivated and enthusiastic 1st level Nurse to lead The Community Complex Care and Community Respiratory teams. The service aims to prevent unnecessary hospital admission and support early discharge through the delivery of a person-centered, multidisciplinary, case management approach for people with advanced respiratory disease, frailty and complex care needs.
The Team Leader will act as a role model to lead and manage diverse teams consisting of Respiratory Clinical Nurse Specialists, Respiratory Nurses and Frailty Practitioners, Physiotherapists and HCSW’s. You will work closely with Multi-disciplinary teams within the HSCP, Acute services and the respiratory MCN to ensure the delivery of safe effective patient centred care.
You will be experienced in all aspects of managing and leading a team, including change management and service redesign. You will have excellent communication, organisational, I.T and clinical assessment skills. You should have substantial post-registration nursing experience for this role. Experience working within a community nursing and respiratory or frailty setting is essential to this role. You will have evidence of recent CPD, be educated to post-graduate diploma/ MSc level or evidence of working towards. You will have a non–medical prescribing qualification. You will be supported to achieve the BTS respiratory CNS competencies and any other training necessary for the role.
The ability to reliably and efficiently travel throughout Fife to various locations and patients homes is essential to this role. Public Holiday working may be in operation (except Christmas Day & New Years Day).
