An exciting opportunity has arisen for an experienced pharmacist to join our Pharmacy team in providing safe, effective and cost-efficient use of medicines to patients receiving social care support. We are looking for a dynamic, highly motivated and enthusiastic pharmacist with excellent communication and organisational skills to take on this new and exciting role.

This role will focus on the development of an integrated approach to pharmaceutical care for patients in their own homes receiving social care support. It will primarily involve completing Realistic Medicine/Polypharmacy reviews to address individual patient needs to ensure safe and effective use of medicines in line with the NHS Scotland Prescription for Excellence and Realistic Prescribing strategies. This also includes working with our pharmacy technicians and Social Care and Integrated Healthcare MDT colleagues to ensure that patients have the appropriate support they need for managing their medicines safely at home. This may involve undertaking home visits to patients to address medication related issues, and working across traditional Primary-Secondary Care boundaries, as well as the health and social care interface.

Applicants should have completed the NES post-registration vocational foundation training programme or equivalent relevant working experience, a postgraduate qualification to Masters or Diploma level in clinical pharmacy or equivalent experience; obtained their Independent Prescribing qualification and have a highly developed specialist therapeutic knowledge and practical skills dealing with a complex range of pharmaceutical interventions in elderly patients. Experience of working with patients and their carers would be an advantage.
