An exciting opportunity has arisen for a Band 4 Cancer Support Worker to work within the upper GI cancer multi-professional team.
The post holders will work alongside the Specialist UGI,HPB and Neuroendocrine Cancers upper GI cancer clinical nurse specialist and the rest of the team to undertake the following:
* provide a single point of contact for discussing questions or anxieties related to their supportive care needs & clinical care from the point of diagnosis Single point of contact via telephone or email for needs to be addressed timeously. CSWs’ will have in-depth knowledge of the nuances of the cancer specific pathways to act proactively in keeping people informed about ‘next steps’, ensuring people receive timely and accurate advice on their appointments, test and results.
* provide tailored support for the patient and their family, based on person centred conversations & holistic needs assessments & delivery of specific interventions particular to the cancer site requirements, providing people with the chance to discuss what non-clinical support may be available for them and their family following a cancer diagnosis
* assist people to understand their treatment plan and expected timelines for treatment delivery
* Commencement of rehabilitation according to local protocols for each cancer and patient
* promote self-manage (aspects of) their condition and access available services as appropriate following discharge
* undertake duties appropriately delegated to them from the cancer clinical nurse specialists so they can undertake complex care
* Undertake immediate referral to Macmillan CAB services to reduce inequities and ensure financial gain is maximised for patients
* Referral onwards to community Cancer Support Workers for locally delivered care & support to enhance team working
* assist in reducing waste in the systems such as missed appointments, duplication of interactions e.g. support sought from CNS and local GP services, as people are better informed. Logistical issues can be addressed proactively particularly given our geography and the need for some patients to travel out with our Health Board to receive their treatments or investigation
* a willingness for a 2 month induction training.
