As part of the Scottish Government Discharge Without Delay agenda and Home First initiative we have an exciting opportunity to join our new multidisciplinary Home Assessment Team which aims to reduce delayed hospital discharges by assessing needs and providing a period of rehabilitation in the home setting.​​​​​​​

The Home First pathway encourages all health and care professionals to ask the questions “why not home, why not now?”at every stage and has been evidenced nationally to result in significantly improved outcomes with reduced risk of deterioration in function through unnecessary prolonged hospital admission. An assertive and renewed approach to positive risk taking, identifying what a person has as opposed to what they need, supports people in maintaining their independence. Home Assessment Teams are currently being established across North Lanarkshire to support this process.

We are looking to recruit an enthusiastic Team Leader with HCPC registration and either an Occupational Therapy or Physiotherapy background to operationally lead the Allied Health Professionals in the North East Sector Team.  Sector bases have yet to be confirmed however post holders will be hybrid working between their community base site, the acute sites and domiciliary visits. The post holder will work closely with a North West Sector Team Leader to ensure North Lanarkshire wide cover with subsequent cross-cover as required.  There may also be a requirement to carry out some work from home. The post holder will also provide significant and valuable input into the acute site Home Assessment Team Responder rota.

A full UK or EU driving license and access to a car is essential.
