We are expanding our Reablement Service and are looking for a motivated Healthcare Support Worker to join our Extended Community Care Team within Helensburgh and Lomond.
Reablement is a goal-focused intervention that involves defined therapeutic work over a 6-8 week period. Reablement uses a strengths-based, person-centred approach which has been shown to lead to improvements in health and well-being. Our Reablement Service, together with the wider Health and Social Care Team, can support timely discharge from hospital and support individuals to remain living at home if, due to poor health or disability, they have increasing difficulty with daily life activities. Reablement promotes choice, autonomy and independence.
The responsibilities of our Reablement Healthcare Support Workers include daily delivery of person-centred Reablement Care to the community of Helensbugh and Lomond.
Our team support and facilitate collaborative goals and action plans with patients to improve independence and safety within their own home.
Previous experience working in Health and Social Care settings would be beneficial but is not essential due to the training and support available. Attributes of a successful candidate include: excellent communication skills, team working skills, adaptability and effective time-management. Good IT skills are also required.
You will be line managed by the Helensburgh and Lomond Occupational Therapy Team Lead.

Access to a car and a full driving licence are essential. Please note these posts include weekend and evening working.
