We are looking for an enthusiastic and motivated individual to join our team as a Single Point of Contact Hub Patient Navigator for our Central Referral Unit. The aim of the service is to improve experience for patients referred urgent suspected or diagnosed with cancer and will be achieved supporting patients in:
• timely access for all appointments (diagnostics, MDT, clinic, surgery)
• signposting - clinical advice /support services.
• timely access to appointments for follow-up post treatment
• provision of Patient Pathway Information
You will be at the forefront continuing set up and development of a new administration process for the service. You will work within the wider cancer Services team and with GPs and Advanced Nurse Practitioners who will be referring patients urgent suspected cancer. You will be contacting patients to ensure they are aware of their appointments and to answer any queries they may have. You will be a source of contact for these patients and their families, helping them to navigate through their suspected cancer pathway, through diagnostic tests to diagnosis and ongoing referral to treating specialist cancer clinical teams or discharge back to GP if no cancer found.
We are seeking people who can exercise their initiative, use independent judgement, and make decisions within a variety of situations.
You must demonstrate effective communication, be flexible, a team player and have the ability to work on your own initiative. Good administrative and clerical skills essential; experience of direct patient contact desirable.
You will work within a multidisciplinary team.
