An exciting opportunity has arisen for a Team Leader within the Front Door Integrated Discharge Team.  A fantastic opportunity to be part of the national programme of ‘Discharge without Delay’ and the Home First initiative. 

Working within the Health and Social Care Partnership as part of the integrated team to ensure timely patient centred patient flow between Acute care at VHK and the wide range of community services including downstream beds, ICASS and Social Care and Social Work settings.

You will have managerial responsibility for the new established Front Door Team (PFC’s/ ARPs/ Senior Social Work Practitioner), supporting the team to undertake comprehensive assessments for patients within the VHK receiving units ensuring the appropriate patient pathway is established for each individual, including the assessment for homecare services, put in place any requirement from external agencies, equipment and support required to facilitate discharge or transfer to another facility. Liaise with MDT and support patients, their families and their significant others.

You will be a Registered AHP/ Nurse/ Social Work or equivalent with a Diploma/Degree in AHP/ Nursing/ Social Work.  A good understanding of the complexities of discharge planning is preferable but not essential.

You should demonstrate extensive clinical and professional practice preferably within care of older adult and complex discharge planning.
