Lead Pharmacist Care of the Elderly

Band 8A, 1.0 WTE, 37.5 hours

(including evening, weekend working, oncall and public holidays commitment)​​​​​​​

St John’s Hospital is a modern teaching hospital that provides a comprehensive and expanding range of services, including a 24-hour Accident and Emergency department and the future home of the National Treatment Centre in NHS Lothian.

We have an exciting opportunity for a dynamic and motivated pharmacist, with a passion in caring for elderly patients, to undertake a leadership role in transforming clinical pharmacy in this specialist area. The clinical portfolio for this post includes: medicine of the elderly, stroke, frailty and orthopaedic rehabilitation. The individual will be pioneering the pharmacist role in multidisciplinary team (MDT) ward rounds as part of the frailty service, building on a proactive role to optimise patients’ medicines and improving their stay in the hospital through effective discharge planning. We are also keen to build on the integration of clinical pharmacy service with primary care to ensure a smooth transition when patients move across different care settings. The postholder will also be participating in the clinical pharmacy team in A&E with particular focus on frail patients.

Candidates should have excellent clinical, organisational and interpersonal skills. Experience in working in a patient-focused environment and proactively influencing prescribing within the MDT is highly desirable. There will also be opportunities to lead on service and personal development roles within the care of the elderly/frailty teams as well as in Pharmacy.

Candidates must have a Master of Pharmacy Degree or equivalent, be registered with the General Pharmaceutical Council (GPhC), and should have completed a Clinical Diploma or Masters in Clinical Pharmacy. Due to the nature of this role, independent prescribing qualification is essential.
