Highland Health and Social Care Services help people from all walks of life, as well as their families or carers. Health and Social Care in Highland provides help and assistance in many situations arising due to illness, disability, old age or poverty.
An exciting opportunity has arisen within Caithness Integrated Teams for a Health and Social Care Coordinators (HSCC).
The HSCC plays an integral role within the Integrated Team gathering referral details from various sources including clients, their family members and other Health and Social Care Professionals. This information is then shared with the multi-disciplinary team in order to prioritise and allocate.
The HSCC provides support to professional staff when dealing with urgent and critical cases in the community.
The successful candidate must have excellent communication skills, good time management and organisational skills and have a flexible approach to their work.
A driving licence is essential for this post and applicants should have access to their own vehicle for work purposes.
This is a full time Fixed Term / Secondment post to March 31st 2024 of 37.5 hours to be worked in accordance to service requirements.
