An exciting opportunity has arisen at Victoria Hospital in Kirkcaldy for a senior health care support worker who is keen to enhance and develop their knowledge and clinical skills in a pivotal band 3 position.
The candidate should have a commitment to acute medicine, patient centred care and supporting effective team working. The post holder will be responsible for coordinating and leading discharge planning, supporting the senior nursing team and helping deliver change within Admissions Unit One. The post holder will be expected to ensure safe and effective care that enhances the patient experience and contributes to the delivery of the organisation's objectives. The post holder must have experience within an acute clinical setting at band 2 level and be able demonstrate an excellent standard of clinical and professional knowledge and skills.
Admissions Unit One consists of 37 inpatient beds and also has 2 busy GP Assessment areas. As a discharge coordinator you will be expected to have an overview of the Unit and support the coordination of capacity and flow through discharge management. We work closely with many multidisciplinary teams and they assist us with enhanced discharge planning.

Hours available for this post are 37.5 hours per week predominantly worked over 3-4 days with occasional weekend working and is on a Fixed Term basis for 6 months.
If successful in your application, you will be offered the opportunity to undertake an SVQ2 or SVQ3 (dependent on band) in Health and Social Care, approximately 9 months after taking up post. It is expected that you complete this qualification, which will be fully funded by NHS Fife. Your Line Manager will provide you with more information once you are in post
