Pharmacists, are you ready for a change or to try something new?   This is an exciting development opportunity for someone looking to enhance their clinical skills and train to become a highly competent General Practice Clinical Pharmacist.  These posts would be ideally suited to a newly qualified pharmacist keen to develop their clinical skills or a community pharmacist looking to focus on this rapidly expanding area of pharmacy.  

Over a 12-23 month period (based on existing experience) you will ‘learn on the job’ by working under the supervision of an experienced team of pharmacists and receive education and training by our Education and Training Team.  There will be opportunity for you to spend part of your training in the acute pharmacy setting to gain skills in this area of pharmacy and an appreciation for the issues faced at the interface between Primary and Secondary care.  Your competency and capability based training programme, based on the Royal Pharmaceutical Society framework, will give you a solid platform on which to go on and build your career within the Team.  There is a requirement within this post to work as an Independent Prescriber and you will have the opportunity to gain your Pharmacist Independent Prescriber qualification if you do not already have this.

Working as part of the Community Health & Social Care (CH&SC) Pharmacy team, you will work in GP practices as part of our stimulating General Practice Clinical Pharmacy service. Our pharmacists and clinical pharmacy team take a leading role in medicines management and medication review as part of the multidisciplinary GP and CH&SC teams. This is a chance for you to make a difference to the people of D&G and to help them live longer, healthier and happier lives by helping them to make best use of their medicines.

The post holder will be based in GP practices and within the award winning pharmacy hubs.  Day-to-day responsibilities might include: clinical assessment and review of patients, performing polypharmacy medication reviews with patients, dealing with medicines related queries, assessing patients ability to manage their medicines (including in the patient’s home), medicine reconciliation and prescribing of medication.  

Joint working with other Community Health and Social Care colleagues will be routine, as well as with the multidisciplinary and professional teams within GP practices, Community Hospitals, Community Pharmacies and newly operational Home Teams.
