This is a good opportunity to join the community staff at a time when our service is developing Home Teams, a new way of delivering our community services, involving all our MDT colleagues working together to get patients out of hospital quicker and to deliver rap around services, to keep them safe till a full assessment has been completed. We will also strive to deliver a rapid response element to Home Teams where we will strive to assess those patients who are in distress/at high risk and prevent their admission to hospital. How will we do this? By
developing our community assets and supporting the community to protect/Support their most vulnerable. Nursing will be a vital part in this, providing HCSW support with reablement and care. Our community staff providing assessments when appropriate, feeding back through the MDT meeting and helping to shape how our Home Team develops.

Community is the place to be learning new skills, learning how to think differently and how to think on your feet, problem solving but knowing you are not alone and have an MDT team behind you to support you and your patient.
Regular travel to various locations occurs daily and therefore a current driving licence is essential.
The Community Nursing Service operates 365 days a year 24/7 so there will be night shift cover. The service does not operate on a waiting list; care needs are prioritised and the team respond to planned and unplanned care. Unscheduled visits and those who require urgent Intervention are responded to timeously.
