An exciting opportunity has arisen for highly motivated, enthusiastic and suitably qualified and experienced registered nurse to join the Interim Care Team in Falkirk Health and Social Care Partnership. The Interim care team provides care to bridge the gap to support people on their initial discharge from hospital and to facilitate earlier hospital discharges. All care is provided with a reablement ethos.

The post holder will be committed to the development of an effective, flexible; person centred Care Team through your ongoing professional development as well as that of the team.
The post holder will have the responsibility for the line management and clinical leadership of the Interim Care Team providing care to a defined patient caseload. The post holder will work closely with the HomeFirst team to support people to be discharged in a timely manner from hospital.
As part of a multidisciplinary team the post holder will have responsibility for assessment of care needs, evaluation of care plans, ensuring the delivery of high quality care to patients.
The post holder will have responsibility and accountability for maintaining both clinical and staff governance.
Excellent time management skills as well as the ability to effectively prioritise workload are essential requirements enabling patients to remain as independent as possible in their own homes.
The post holder will have a full induction and orientation programme as well as ongoing support to assist in continued professional development.
The duties of this post require the successful candidate to be a member of the Protecting Vulnerable Groups (PVG) Scheme. More information on this scheme can be found at www.disclosurescotland.co.uk
The post holder will be required to work over 7 days per week
Ability and willingness to travel throughout the area of service delivery in a timely manner is essential for this post.
