An opportunity has arisen for a Cardiac Clinical Nurse Specialist to join the Fife Community Cardiac Specialist Nurse Service. This post will initially focus on supporting patients with heart failure however further developments are planned and over time the role will widen to include the support of various other cardiac conditions.
Heart Failure Advanced Clinical Nurse Specialists (HF ACNS) will support you through appropriate delegation of work and support to create individual management plans for patients with heart failure. The service aims to provide evidence based & specialist care, prescribing evidence based treatments, supporting self management, improving quality of life and reducing hospital admission / re-admission.
Over time you will support people with other cardiac conditions to recover from their cardiac event. This will be done by undertaking individual clinical risk assessments and supporting patients in their decision making to self manage their condition. You will be able to identify the most appropriate programmes of care for individuals. This will be done through working in partnership across primary and secondary care and includes agencies and organisations outside the NHS to support people with heart disease in their individual rehabilitation and health behaviour change.
As a Cardiac Clinical Nurse Specialist you will be a first level Registered Nurse with current NMC registration and substantial cardiology experience, you will provide a high quality, safe patient centred and effective clinical nursing service. You should have a non medical prescribing qualification.
You should evidence excellent communication skills, clinical assessment and decision making skills. You will work as a member of the wider Health and Social Care Team with the ability to work autonomously utilising your own initiative, clinical judgement and decision making.
As this is a community post you will also be required to travel across Fife
