As an integral member of the multi-disciplinary team the post-holder will work in partnership demonstrating the compassionate, caring behaviour’s required to fulfil the key functions of the Care Home Liaison Nurse within the Older People’s CMHT. The post holder will practice autonomously and will have responsibility for the assessment, planning, implementation and management of care needs and associated risk factors of Older People with Mental Health problems who are Care Home residents. Treatment will be delivered using validated psychological approaches and the use of integrated working to meet the complex needs of this client group. The post holder will be required to undertake duties without direct supervision in addition to supporting and supervising work undertaken by unregistered nursing staff. The post-holder will proactively promote partnership working with patients, their carers and other professionals/ agencies in line with legislative frameworks, agreed standards and clinical policies. This post predominately involves working in designated Care Homes however, the post holder will be expected to undertake duties within the wider CMHT should clinical demands arise. The role includes providing expert clinical advice, practical support and specialist education to other professional groups and Care Home staff regarding the mental health needs of this patient group as well as student nurses.  Car driver is essential.  The shift pattern for this post is Monday to Friday 9am to 5pm. Informal contact: Gail McSeveney Nurse Team Lead on 0141 232 6660 or Gail.McSeveney@ggc.scot.nhs.uk
