Job Advert

This post will support the work of the Discharge Hub at the Victoria Hospital.  It involves comprehensive assessment and planning for patients who are in the Victoria Hospital and complex planning for their discharge.  It involves daily involvement with all the wards at the Hospital and all partner agencies.  You will need to bring a wide range of experience from across Health and/or social care with a background in Nursing or AHP.  

This is a fast paced and exciting post working in a diverse and often changing environment.  You will need to have both an organised and flexible approach to work in this post.  A good team player who is confident in their role.  You will have a relevant qualification and experience in a relevant area.  You will possess excellent communication and organisational skills; demonstrate evidence of effective multi-agency working; and be able to work on your own initiative, plan and manage own workload but also function well within our team. Advanced knowledge and skills in the use of Microsoft Office and associated databases (e.g Excel) are also required for this post.
