Band 7 Advanced Nurse Practitioners or Advanced Clinical Practitioners

An exciting opportunity has arisen for a permanent Band 7 Advanced Nurse Practitioners or Advanced Clinical Practitioners within the Aberdeen City HSCP Hospital @ Home (H@H) team based on Woodend Hospital site. Applications are encouraged from both nursing and allied health professional disciplines.

Hospital at Home provides intensive, hospital-level care for acute conditions that would normally require an acute hospital bed, in a patient's home for a short episode through multidisciplinary healthcare team.  We currently work 7 days/week till 8pm.  We continue to grow a substantive workforce and prioritise the wellbeing and safety of our staff, patients and their families.

Join a motivated, collaborative workforce focused on admission avoidance and active recovery, increasing capacity and resilience to deliver a safe, high quality service in the patient’s own home. 

We are looking for a motivated team member, educated to master’s level qualification or with evidence of equivalent experience in Advanced Practice to provide the clinical assessment, diagnosis and management of patient conditions.  Work in partnership with patients and families, utilising high levels of decision making and clinical judgment with specialist support and advice provided.  The ANP/ACP role encompasses that of expert clinician, educator and auditor.

Previous experience within an acute setting and with frailty, acute medicine or respiratory patients would be desirable.

Ability and willingness to travel throughout the Aberdeen City area is essential for this post.​​​​​​​ The successful candidate must therefore own or have access to a car for work purposes and hold a full UK driving licence.

An attractive relocation allowance of up to £8,000 is available to successful candidates
