NHS Fife has a team of breast cancer specialist nurses that provide support, advice and consultancy to patients with breast cancer. An exciting opportunity has arisen for a band 4 patient navigator to join our team in order to support the high level of service we offer.
This post will be busy and varied. You will offer the following:
- Coordinate care by providing a single point of access,
- Provide advice and refer on or sign-post to other sources of support
- Coordinate diagnostic and monitoring through appointments and investigations
- Coordinate face to face appointments for those with complex results.
- Discuss with patients issues relevant to their health and social care and provide support where possible or escalating relevant clinical teams if necessary
As well as excellent communication skills, you will be confident.
You will have complete support from the team in order to meet your needs to excel in this role.
