What you will do:

Post 1: Speech and Language Therapist

  • You will work as part of the Speech and Language Therapy (SLT) Service​​​​​​​ delivering independent working with a complex and highly specialist clinical caseload and/or workload, demonstrating highly specialist knowledge underpinned by theory and practical experience
  • Provide a lead role in specialist area, delivering and supporting the delivery of evidence based practice with appropriate interventions from a range of options
  • You will be accountable for own professional actions within codes of practice, professional guidelines and policies including defined occupational policies
  • Be responsible for assessing, diagnose and treat own caseload and maintain associated records
  • You will work in consultation with patients, clients, families, carers, healthcare staff and Multi-Disciplinary/Multi-Agency Teams
  • Represent the specialist contribution of Speech and Language Therapy Services and advocate for the needs of a highly specialist caseload in range of multidisciplinary/multi-agency forum within and out with the organisation as required by the Service Lead
  • Proposes policies, procedures, or service delivery changes for specialist area, considering impact on other service areas and professions
  • Identify, lead, and participate in a range of agreed clinical and non-clinical activities relating to service objectives with a designated portfolio of service development activities
  • Provide teaching within own and other professions
  • You will deliver a range of supervision roles for Band 2 Healthcare Support Workers, Band 3 Senior Healthcare Support Workers, Band 4 Assistant Practitioners, and less experienced SLTs and students

Post 2: Dietitian

  • To work within the 7 day CAU Team providing a proactive expert clinical assessment and treatment to people who are assessed and identified as requiring dietetic intervention. Making timely decisions regarding their management, which involves triage of patients  independently, making onward appropriate referrals to Consultants, clinics, interdisciplinary and interagency professions including community AHPs and to discharge patients directly from in-patient Dietetics if appropriate.
  • To act as SPOC (single point of contact) for Hospital @ Home Service for triage, direct or indirect intervention/onward referral to appropriate dietetic clinician/team as required.
  • To treat and manage a large and highly complex clinical caseload across multiple specialties.
  • To lead Dietetic 7 day CAU training and provide clinical support to peers, supporting the escalation process within the 7 day CAU.
  • To work alongside the 7 day CAU interdisciplinary and wider interagency teams using advanced/highly developed skills to aid assessment, management and treatment of people with highly complex conditions e.g. recognising the need for and recommending further investigation in line with protocols and escalation to appropriate interdisciplinary and legislative forums.
  • To act as Dietetic clinical lead for patients with specific conditions, giving highly specialist advice and clinical supervision to other health, social and third sector staff, students, service users, and carers.
  • To assess and manage significant clinical risk within own caseload – this involves the identification of serious conditions and the necessary liaison with all health care professionals, which may involve onward referral.
  • To educate and train members of the health & social care uni-professional and multi-agency teams including students.
  • To act as a source of clinical advice for the health & social care uni-professional and multi-agency teams including students.
  • To lead clinical audits and participate in local & national research projects. Participate in Clinical Governance activities associated with the 7 day CAU Service.

Post 3: Occupational Therapist 

  • You will work within the 7 day Front Door ED/CAU Team providing expert clinical assessment and treatment to people who are referred. Making timely decisions regarding their management, which involves triage of referrals independently, making onward appropriate referrals to Consultants, clinics, interdisciplinary and interagency professions and to discharge patients directly from Occupational Therapy.
  • Treat and manage a large and highly complex clinical caseload.
  • To lead Occupational Therapy 7 day Front Door ED/CAU training and provide clinical support to peers, supporting the escalation process within the 7 day Front Door ED/CAU.

Knowledge, training and/or experience required to do the job:

Post 1: Speech and Launguage Therapist

  • Relevant University Degree or equivalent qualification in Speech and Language Sciences
  • Certificate to Practice from Health Care Professions Council (HCPC)
  • Evidence of further theoretical training and experience to masters level or equivalent within a specialist area, and demonstrable ability work autonomously
  • Extensive experience within a variety of clinical areas
  • Ability to demonstrate leadership skills, management experience, or advanced practice
  • Evidence of service improvement including successful application and completion using quality improvement methodology
  • Extensive experience of working in NHS, Local Authority or a caring setting
  • Excellent interpersonal skills.
  • Relevant digital competency
  • Car driver essential
  • Ability to work independently ​​​​​​​
  • Extensive experience of working with individuals and their families in a supportive role

Post 2: Dietitian

  • MSc/BSc in Nutrition Dietetics or post registration diploma in Nutrition and Dietetics
  • Requires to have post graduate training in relevant speciality, or experience equivalent to a Masters Level.  This would include inpatient and outpatient experience
  • Must have substantial and varied clinical experience of working in a relevant speciality at a specialist level.   
  • Current Health and Care Professions Council Registration
  • Evidence of excellence in clinical knowledge and skills.
  • Excellent communication skills particularly with multi-agencies.
  • Evidence of clinical leadership and supervision.
  • Committed to team & personal development with evidence of self-directed learning.
  • Excellent team worker.
  • Excellent interpersonal skills
  • Ability to work independently, using initiative, and to cope well under pressure.

Post 3: Occupational Therapist

  • Pre-reg MSc/BSc in Occupational Therapy or equivalent
  • Requires to have post graduate training in relevant speciality, or experience equivalent to a Masters Level.  This would include inpatient and outpatient experience, and may also include membership of Royal College of Occupational Therapy Special Interest Groups
  • Must have substantial and varied clinical experience of working in a relevant speciality at an advanced level.   
  • Current Health and Care Professions Council Registration
  • Evidence of excellence in clinical knowledge and skills.
  • Excellent communication skills particularly with multi-agencies.
  • Evidence of clinical leadership and supervision.
