The new Integrated Reablement Team and RAPID service in Clackmannanshire and Stirling HSCP will be instrumental in avoiding hospital admission where appropriate, supporting people to return home as quickly as possible where admission has been required and delivering on issues that people tell us really matter to them to enable them to live well in their community.
The new service will give us the opportunity to offer the post which best suits your career development. Full time posts are static within the team or there may be the chance to gain experience in the wider AHP Rehab Services within the HSCP as opportunities arise. If part time is right for you then we can offer this within the service. Weekend working as part of a rota is included in these posts.
Working as part of the ReACH Clacks and Stirling team will offer you the chance to be at the heart of community rehabilitation services but also be involved in transforming how we support discharge from hospital alongside our social care colleagues. As this will be building a new service delivery model you will gain experience in Quality Improvement and be actively engaged in all aspects of Care Assurance.

As a Band 6 Occupational Therapist you will work as part of a multi-disciplinary team, manage your own complex caseload, using evidence-based practice and person-centred principles to assess, plan and implement evaluations. You will have the responsibility to supervise more junior staff, therapy assistant practitioners and students, you will also play a key role in educating partnership colleagues. This community role offers a well supported structure within an experienced and dedicated team of AHPs. Ability and willingness to travel throughout the area of service delivery is essential for this post as this element of the service is community based.
Registered Occupational Therapist with valid HCPC Registration
Educated to Degree Level
Comprehensive knowledge of clinical guidelines and standards within the specialty/area of practice.
The post holder will possess excellent team-working skills and have the ability to motivate others and work using their own initiative
Excellent listening, communication, and interpersonal skills.
Effective time management skills
Working knowledge of basic information technology.
