What you will do:
In consultation with the AHP Senior Manager and General Manager, you will take the lead role in defining, and shaping clinical pathways within the relevant model of care.
To provide non-medical clinical leadership and delegated responsibility for leading the MDT, provision of professional advice and managing complex clinical decisions.
To provide a focus for clinical and professional leadership and advice across relevant services including acute, rehabilitation, primary care, HSCP services ensuring that standards of care practice reflect professional and organisational standards.
You will contribute to the care of people across a number of settings through working closely and in a professional advisory capacity with teams, including acute in-patient areas and community settings.
The development of a training plan to support the development and roll-out of the model of care, based on an analysis of training needs and reflecting the development of new roles and workforce planning
You will be an expert in the relevant area, demonstrating a sound knowledge of acute, rehabilitation, primary care, HSCP services.
You will contribute to ensuring clinical supervision is available to staff working within the relevant areaThe AHP Consultant will have a key role in ensuring the use and sharing of best practice, evidence based care, clinical audit and evaluation across the service.
You will work with stakeholders to redesign clinical pathways to provide alternatives to admission, reduce delays and ensure seamless care for individuals at interfaces.)

Knowledge, training and/or experience required to do the job:
- Dip/BSc in relevant profession.
- Master’s degree/research degree/PhD or working towards.
- Evidence of post graduate qualification/continuous professional development relevant to the post.
- Non-medical prescriber qualification desirable
- Proven experience of being responsible officer for clinical patient pathway of care
- Demonstrable success in leading complex change and service improvement.
- Proven track record in research and audit activity.
- Excellent communication with well developed negotiation and influencing skills.
- Experience in local and national strategic service design.
- Experience in undergraduate teaching, practice education and contributing to the education of care providers.
