An exciting opportunity has arisen to work within the Chest Heart Stroke Scotland (CHSS) Community Respiratory Team, a brand new service being created for the benefit of the whole of Grampian. Serving patients with both acute and long term respiratory illness, this newly created team will work alongside existing services to deliver more Respiratory specific care to patients in community settings.
You will have a key role in supporting and managing Band 4 CHSS CRT colleagues and will work with the support and supervision of a team of Advanced Practitioners, GPwSI and Consultant colleagues. Applicants will have an excellent knowledge of respiratory conditions and will have proven leadership skills. You will work autonomously delivering complex care for patients with acute and long term Respiratory conditions and will require excellent communication skills as you will have professional responsibility to liaise with teams in primary care, community care and the acute sector. You will also liaise with other Healthcare Professionals such as physiotherapists, pharmacists and the oxygen service to deliver integrated care as part of Grampian wide re-design of respiratory care.
As a new service you will also have a key role in collecting data for clinical audit and clinical effectiveness reviews, seeking feedback from patients and service users, and in shaping the development and future of the CHSS Community Respiratory Team
