We also provide regional and national services across a range of clinical specialties. The vision for the Pharmacy & Medicines service is to deliver clinically led, person-centred services to ensure patients across Lothian get the best from their medicines and pharmacy services.  We are focussed on creating an environment where each person in the team has the opportunity to flourish, develop and make best use of their skills. East Lothian Community Hospital provides services to elderly patients, including acute admission, assessment and rehabilitation and to patients with mental health problems including out-patient and psychiatry of old age in-patients. The postholder will provide a high quality clinical pharmacy service to this group of patients. Key areas for patient focused pharmaceutical care include medicines reconciliation, systematic identification and resolution of individual pharmaceutical care issues, provision of seamless care, provision of a medicines information service and patient counselling. This is a new post due to a new orthopaedic rehabilitation ward opening in the hospital. The successful provision of all these services relies on working within a multidisciplinary team of which pharmacy is a valued member. The postholder will also have close links with other members of the clinical pharmacy team in NHS Lothian. We are looking for highly motivated, enthusiastic pharmacist to join the pharmacy team delivering the highest quality pharmacy services to these patients. The post holder will contribute to the development and delivery of the clinical pharmacy service and will be a key member of the multidisciplinary teams. Candidates will have experience of pro-actively influencing prescribing within a clinical setting, and will have gained experience working in a patient focussed environment using their knowledge and skills to improve patient care and promote optimal outcomes from medicines. Candidates must have a Master of Pharmacy Degree or equivalent, must be registered with the General Pharmaceutical Council (GPhC), and should have completed or have equivalent competence as mapped to the Post Registration Foundation Pharmacist Framework available at:
