An exciting opportunity has arisen within the Care at Home team to support and supervise the enablement care at home team on a day to day basis, with the focus of maximising independence and supporting individuals to remain at home. To work effectively with the integrated team, to work autonomously and have continued responsibility for managing a constantly changing caseload and working to provide a high quality, need led, evidence based and individualised care packages that obtain a positive outcome for the client group. To ensure that the provision of high quality services reflecting SSSC standards.
