An exciting opportunity has arisen within AHSCP Primary Care Team of wellbeing and enablement assistant practitioners. We are looking for individual/s that enjoy working with people in a community environment helping to deliver safe, effective, person centred care and support for patients and carers. The post holder/s will work on a rotational basis across the NW Locality covering Forfar and Kirriemuir between the Community Nursing Team, Enablement and Response Team (ERT) and Occupational Therapy/Physiotherapy service which will assist providing excellent links within the 3 services.


The successful candidate’s will be expected to work flexibly to meet the needs of the services which will mean shift work between 8am and 10pm. A requirement of the post is a willingness to travel across the geographical area of Angus Health & Social Care Partnership


Your role will involve carrying out a variety of technical, therapeutic and clinical activities for a designated case-load within an agreed competency framework in a range of settings e.g. long term conditions care, continence care, phlebotomy.  You will undertake on-going assessment, planning and evaluation of care.


On a day to day basis you will be part of a team and have proximal supervision from a registered practitioner.  You will work in partnership with individuals, carers, families, professional health staff and other agencies in order to support patients and their families to promote independence.


You should be educated to diploma level in a subject relevant to post, i.e. SVQ, professional development award at level 7/8 or equivalent vocational experience in a healthcare setting.  You should have effective communication skills, competent IT skills and are able to work autonomously with individuals and groups. 


You will be required to achieve the mandatory Induction standards for Healthcare Support Workers in Scotland and to comply with the Code of Conduct throughout your employment
