An exciting opportunity has arisen within the Caithness District Team for an experienced Band 6 Nurse to join the Hospital at Home team. 

Funding has been secured from the Scottish Government to develop and trial a Hospital at Home service in Caithness, giving us the opportunity to design a service that will deliver acute level care, in patient's homes, in a remote and rural area.  This is an opportunity to work in an innovative and high-profile field, to shape a service that could be replicated nationally and internationally, and to make a difference to patients in Caithness.

Hospital at Home will sit within the developing Caithness Local Care Model, which aims to provide patients with complex needs and at high risk of hospital admission with dedicated care by a multi-agency, multi-disciplinary team. This approach aims to:  

  • Deliver person-centred, coordinated, seamless care;
  • Work in partnership with patients and their families to maintain independence and optimum health for as long as possible;
  • Help patients access support and resources;
  • Anticipate and plan for patients’ needs;
  • Have all health & social care skills in one Multi-Disciplinary Team; and
  • Deliver a streamlined process with a fast response time and quick decision making.

 This is an exciting opportunity to be involved in the setting-up and development of an innovative new service as part of the Caithness Redesign. We are looking for forward thinking, passionate staff who can take a positive risk management approach to support people to remain at home when acutely unwell, within our Hospital at Home service.  The post will involve working as part of a multi-disciplinary team to assess, plan, implement and evaluate specialist nursing & medical care. You must be registered with the NMC and have experience in an urgent, community or acute setting. 

This is a full time fixed term / secondment post of 37.5 hours to March 2024, with a potential for extension, to be worked in accordance with service requirements.
