GP with Extended Role: Hospital at Home
GP Salary £69,993 - £104,469 per annum
Plus £1,279 Distant Islands Allowance per annum
Full time 40 hours + OOH weekend sessions
Fixed term post or Secondment until March 2024
(Part-time or Job Share would be considered)
An exciting opportunity has arisen for a GP to work with the developing Hospital at Home (HaH) team based at Western Isles Hospital. This has been made available with fixed term funding from Health Improvement Scotland for one year from the date of commencement.
The objective of the pilot is to provide Clinical Leadership, Clinical input in WIH and the community, and support and training for the substantive team of nurses and HCSW’s. The role has a large strategic element in helping to create a culture of supported admission avoidance and early facilitated discharge with patients remaining under the virtual HaH ward and staff, as well as support and clinical guidance on expansion of the service to the Southern Isles.
We are looking for an individual who understands the current challenges faced within the NHS in Western Isles and is willing to innovate to help patient’s get the best possible care in their home environment. The individual must be willing to assess new presentations in the community as well as provide in reach into patient’s felt suitable for the service in WIH working with the Consultant and Nursing staff to ensure safe and effective care. A full UK driving licence is necessary.
