As part of Unscheduled Care within the whole system of Community Mental Health Services the post holder will work closely with all disciplines/agencies. The post holder will provide a service for individuals with complex mental health, physical and social care needs who present in crisis who may require additional home treatment/care intervention over a 24hr period, also providing a specialist mental health service into the Mental Health Assessment Units, Emergency Departments and inpatients services within the General Hospital setting. The post holder is expected to provide specialist advice to multi-disciplinary staff including consultant medical staff. The service provides assessment and access to mental health inpatient beds over a 24hr period throughout GG&C. This includes arranging hospital admission for a patient should this be required. Due to the rapid response nature of Unscheduled Care, the post-holder will assist the Senior Unscheduled Care Nurse in response to patient need. The post-holder is responsible for prioritizing their own workload and utilising resources appropriately to ensure all patient needs are met. However, they will be accountable to the Senior Nurse for the management of their workload. The Senior Nurse will delegate responsibility to the post holder ensuring that the right level of competency of their role is matched to duties being undertaken. The post holder would be expected to make clinical judgements and decisions in the absence of the Senior Nurse, which will be supported through line Management Supervision. The service operates over a 24hr period and has a number of bases across NHS GG&C.
