What you will do:

As a member of the Primary Health Care Team and working collaboratively with the Clinical Team Leader, the Community Charge Nurse will assist in the implementation of local and national strategies and will provide clinical and managerial leadership to the nursing team. 

Working in partnership with other health professionals, the local authority and voluntary agencies, you will have overall responsibility for the planning, delivery and evaluation of nursing care, including health improvement initiatives, for a defined caseload.  You will be responsible for the appropriate use of resources, including equipment, supplies and manpower within the department and have autonomy in clinical decision making.

Knowledge, training and/or experience required to do the job:

You will be a First Level Registered General Adult Nurse with Specialist Practitioner Qualification or other post graduate community nursing qualification and current NMC registration.

Excellent leadership skills including managing a mixed skill team and teaching and mentorship skills.

You will have competency based clinical skills and assessment skills relevant to this area of practice.  Advanced skills in palliative/end of life care in the community setting are also required.
