This is an exciting opportunity for an experienced nurse to join the Specialist Palliative Care (SPC) team in Fife. As part of the largest service redesign in Fife HSCP delivering SPC, we collaborate with and support other teams to deliver primary palliative care across the Kingdom. We are now looking for a highly motivated and dynamic Clinical Nurse Specialist with knowledge of future directions in palliative care to work within the team.
The outreach service consists of the Community and Hospital Support Team. You will be part of a team delivering and supporting holistic care to the people of Fife with advanced life limiting illness. Through an assist, advise and educate delivery model, we aim to meet the Scottish Governments Strategic Framework for Action 2017 recommendations that “everyone who needs palliative care will have access to it”.
Our service is contacted by professionals through the Single Point of Access (SPOA) which is our central point, enabling immediate specialist advice for all areas of SPC including Outreach clinical advice and Essential direct care, Acute Hospital and Allied Health Professional support and Hospice inpatient referrals.
Your remit includes leading and participating in daily huddles, discussing, and allocating both planned and responsive care to meet patient needs and you will contribute at the weekly Outreach Multi-Disciplinary Team meeting. You are a source of professional advice and expertise to patients and families and colleagues within the Health &Social Care Partnership and the Acute hospital. You will be expected to travel throughout Fife working in all areas of the service and is essential you can travel in a timely manner throughout Fife.
You will assess, treat, prescribe, and direct further management including sign posting, referral to other care pathways, supporting discharges from inpatient to community setting or liaising with colleagues when admissions to a variety of settings are required.
We have a variety of visiting students and learners who access the service, as a practice assessor/ supervisor you will support individuals during their visits or placements.
You will be part of a multi-disciplinary team and expected to take part in both continual professional development at an advanced level and take part in service development alike.
In collaboration with the Essential Care Charge Nurse, you will assist the Team leader ensuring day to day service delivery is maintained and provide support to junior staff across the service.
