Sources of Support works alongside patients in Dundee GP Practices whose physical and mental health and wellbeing is impacted by social and economic issues and personal circumstances. The service offers non-medical interventions and co-ordinated care to help improve patients’ quality of life, whilst alleviating the pressures placed on practices due to non medical issues.

An exciting opportunity has arisen within the service and we are looking for a highly skilled practitioner to join our team as a Primary Care Link Worker (Social Prescribing). Our team consists of 8 Primary Care Link Workers and 2 Primary Care Associate Practitioners (support workers). The post holder will work with patients face to face in GP practices and health centres assessing their needs and case managing their care. They will establish themselves as part of the wider GP practice team, covering up to 3 GP Practices and working with other service providers within the GP contract.

The successful candidate will have a caseload of 35 patients with complex needs and case manage the needs of the patient for up to 20 weeks to help them achieve their identified goals. The post holder will work alongside the patient, utilising a range of different approaches to affect change and facilitate access to appropriate services with a view to improving physical and mental wellbeing, and quality of life. Advocacy and liaison with primary and secondary care, statutory and third-sector services is a key feature of the role.

This is a fast paced role and candidates will have excellent communication skills and must be able to demonstrate working at a senior practitioner level, working with people on a one to one basis who have complex social, economic and personal circumstances. Candidates should have experience of managing a complex caseload where highly skilled decision making is essential to coordinating patient care, liaison and advocacy with other services,  managing risk and  the ability to work autonomously. Key to the role is the robust understanding of the challenges faced by patients living in deprivation. Experience of applying the principles of person centred working and therapeutic interventions are also required, together with the experience of multidisciplinary and collaborative working with partners within a range of work settings.

Applicants must be educated to degree level in a relevant area, preferably with evidence of post-graduate training or qualifications.

You will be required to achieve the mandatory Induction standards for Healthcare Support Workers in Scotland and to comply with the Code of Conduct throughout your employment.
